SUMMARY
The Contra Costa Adult Continuum of Services (CCACS) program is a public/private partnership with Contra Costa Health Services, Health, Housing and Homeless Services and Heluna Health. Our mission is to provide safe, interim housing with comprehensive services that assist homeless adults in securing permanent housing that will end their homelessness. We respect human dignity and believe housing is a right that is supported through advocacy and collaboration (www.cchealth.org/services.homeless).
The Contra Costa Youth Continuum of Services (CCYCS) is a program of Contra Costa Health Services - Housing and Homeless Services Division, in partnership with Heluna Health that provides drop in services, interim shelter, and transitional housing for homeless and at-risk youth ages 18 to 24. The guiding philosophy of CCYCS services is that every youth has strengths and the ability to make positive changes in their lives. We value the diversity, determination, resiliency, and strength all youth bring to our doors.
The CORE program is a public/private partnership with Contra Costa Health Services, Health, Housing and Homeless Services and Heluna Health. The Coordinated Outreach Referral, Engagement (CORE) program works collaboratively in small teams to engage and stabilize homeless individuals living outside to identify plans to end their homelessness permanently. CORE Outreach Coordinators are responsible for county-wide mobile and street outreach including the development and facilitation of outreach strategies that will support those living outside to find and sustain housing. CORE Outreach Coordinators play leadership roles among a multidisciplinary team which serves to support and stabilize individuals moving from street to service. CORE teams operate various hours.
The Supportive Housing Programs (SHP) and Rental Assistance Programs (formerly Shelter Plus Care) are programs of Contra Costa Health Services Health, Housing and Homeless Services Division, in partnership with Heluna Health.
AA / AS degree in Human Services, Psychology, Counseling, or related field AND two years of full-time direct experience working in a professional capacity with homeless, behavioral health or medically complex populations; OR five years of full-time direct experience working in a professional capacity with homeless populations with a high school diploma or equivalent.
