Napa Post Acute is seeking a skilled and experienced MDS Coordinator (LVN or RN) to join our dedicated team in Napa, CA. Position: MDS Coordinator
Salary Range: LVN - $90,000-$100,000, RN- $110,000-$120,000

- Current California LVN or RN license.
- Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.
- In-depth knowledge of MDS 3.0, care planning, and regulatory guidelines.
- Strong organizational skills with a keen attention to detail.
