The Nurse LVN/LPT is responsible for providing comprehensive nursing services to patients within the program. This includes overseeing and addressing patients health needs, delivering direct care, administering medications, and offering health education. The role also involves assessing both the physical and behavioral health of patients, intervening as necessary, and collaborating with the treatment team, physicians, and community agencies to ensure effective patient care. Additionally, the position includes maintaining accurate documentation, ensuring medication adherence, and providing ongoing support and education to patients and their families. The Nurse LVN/LPT will work within clinical guidelines, uphold productivity standards, and contribute to the overall success of the program.
How will I contribute
Oversee patients' health needs by providing direct care, health education, and support as needed. Assist physicians during scheduled medication appointments and provide oversight on medication management. Assess patients physical and behavioral health, intervening when necessary to address issues. Record and maintain accurate, timely documentation in patient charts, including but not limited to: Attendance for billing purposes, Blood pressure and weight, Medication administration details. Administer prescribed medications and injections accurately and ensure adherence to treatment plans. Plan and implement health and hygiene education for both patients and staff. Engage patient family members in treatment planning and provide translation support when needed. Collaborate with the pharmacy to manage medication refills and provide oversight on medication monitoring. Coordinate with program staff, physicians, and community agencies involved in the patients care. Provide proactive follow-up to ensure patient adherence to medication plans. May require occasional evening hours based on program needs. Ensure compliance with clinical rules and regulations, maintaining a productivity standard of 100 billable hours per month. Complete clinical documentation on the same day as services are provided, with a target of 80% compliance. Provide translation support for Spanish-speaking consumers, as required. Contribute innovative ideas to improve the program's effectiveness and outcomes. Use electronic record-keeping systems, including Smart Care Scheduler, Program Notes, and Clinicians Home Page, with proficiency. Achieve all performance standards and participate in ongoing cultural humility training (4 hours per year). Serve on designated committees and boards as directed by the supervisor. Maintain an active status with the California Board of Nursing as a Licensed Vocational Nurse (LVN) or a Licensed Psychiatric Technician (LPT), ensuring compliance with all relevant regulations. Work collaboratively with local Board and Care facilities to support consumer needs and prevent unnecessary higher-level placements. Adhere to the compliance code of conduct and all related policies and procedures, including state, federal, and program-specific regulations. Perform other duties as assigned to support the overall functioning and success of the program. May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible.
What I bring
High School Diploma (or Equivalent) Valid LVN license in the state of practice, Adult First Aid and CPR Class C Driving License Minimum of 1 year prior experience working as a Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) in a behavioral health or psychiatric setting. Experience in administering medications, conducting patient assessments, and providing direct care and health education is essential. Knowledge of electronic health records, medication management, and the ability to work effectively with multidisciplinary teams. Experience in psychiatric nursing, including assessing and managing behavioral health conditions, is required. The role also demands a solid understanding of medication administration, health monitoring, and patient education. Familiarity with mental health treatment plans, crisis intervention, and collaboration with healthcare professionals and community agencies is essential. Knowledge of relevant healthcare and behavioral health regulations and compliance standards is also necessary. Must have the ability to assess and respond to the physical and behavioral health needs of patients effectively. Strong organizational and communication skills are essential for maintaining accurate records, collaborating with the healthcare team, and providing patient education. The role also requires the ability to administer medications accurately, manage health monitoring tasks, and support crisis interventions when necessary. Must have the ability to conduct home visits, work independently, and collaborate effectively as part of a team in a fast-paced, dynamic environment. Ability to obtain credentialing through the local County Managed Care Department. The role requires compassion, empathy, and a strong commitment to patient care. It demands attention to detail, responsibility, and professionalism in challenging situations. A collaborative mindset and the ability to work effectively within a team are essential, along with adaptability to changing patient needs and work environments. Strong interpersonal skills and the ability to build rapport with patients, families, and colleagues are also important for success in this role. Bilingual skills (Spanish/English) are a plus.
