Licensed, Behavioral Health Clinician - SSUD/MAT - Covington, GA. (Personal Services)
View Point Health MAT services are designed to meet the American Society of Addiction Medicine Patient Placement Criteria for the Treatment of Substance Related Disorders. MAT services will be accessible to individuals within View Point Healths catchment area of Gwinnett, Rockdale and Newton Counties with office based access points in Gwinnett and Newton Counties. Each location will serve approximately 25-30 individuals. MAT services will be set up and delivered within a recovery-oriented system of care and will generally be provided as an adjunct (i.e., secondary, or tertiary program) which will be integrated with other medical and/or primary treatment programs. We are looking for a Licensed Behavioral Health Clinician to join our multi-disciplinary team to conduct intakes/assessments, facilitate groups, conducts discharge planning as well as carry a caseload and conducts case management & care coordination.
Under supervision learns to perform psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.
Duties & Responsibilities:
Provides initial program orientation to clients presenting for treatment of BH, AD, and related personal problems.Assesses clients treatment needs, completes biopsychosocial assessments, and assists the client in developing appropriate SMART treatment plans.Facilities groups with clients with topics tailored to early recovery skills (i.e., psychoeducation, skills and some process groups)Provides group, family, and individual education, skills-based training and supportive counseling to clients and their families/significant others.Clinician may also educate families/significant others on the addiction and recovery process.Meets with clients at established intervals to discuss progress toward goals.With input from the clinical lead as needed, identifies unmet needs, reviews available resources, and suggests modifications to the treatment plan.Writes timely progress notes that relate directly to the treatment outcomes described in the treatment plan within 24 hours.Provides crisis intervention services as needed and facilitates transition of care for clients needing to access an Emergency Receiving facility.Completes discharge summaries and other documentation and reports accurately and timely.Assembles, analyzes, and interprets new/updated information about the client to reassess treatment needs.Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated.Maintains a smaller caseload of 10-15 clients.Ensure clients with suicide risk have all necessary suicide prevention measures in place including the treatment plan, ensuring appropriate levels of observation, completing the safe environment prevention plan.Completes treatment authorizations and re-authorizations within required time frames.Serves as case manager to facilitate and ensure communication, linkage, and continuity of care between clients, physicians, other organizational components, community resources and service providers. Documents collateral contact on all clientsEstablishes linkages and coordination of services with community agencies.Develops and maintains cooperative working relationships with community service providers.Informs clients and families of the availability of community advocacy and support groups as well as services within View Point Health
Minimum Qualifications:
Master's degree in a related field from an accredited college or university AND Fully or Associate licensure (LCSW/LPC/LMSW/LAPC). Experience working with individuals who are diagnosed with substance use disorder. Experience in social service delivery and ECR.
Preferred Qualifications:
Experience with Carelogic or other EHR/EMRPrevious experience working with MAT, SUD and/or co-occurring disorders.Excellent communication & interpersonal skills; enjoys working w/ others; is flexible, quick to adapt to changing, fast-paced environment; MUST HAVE great organization, time-mgmt. & documentation skills.
Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.Other duties as assigned.
