View Point Health's Centerville Play Therapy & Resilience Training Center offers culturally, linguistically, and developmentally appropriate education & practice of resilience skills through play and family therapy focusing on infants through age 17 and their family. Engagement & Care Coordinator provides direct care to advance program and service engagement by identifying needs related to social determinants of health coordinating care for children and their family including referral, linkage, monitoring and follow up. Duties are performed in a fast-paced environment that requires abilities to demonstrate organization skills, professionalism, solution seeking skills, and motivation to serve the community.
Under broad supervision, provides a broad range of social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties. Performs lead worker functions.
Duties & Responsibilities:
Provides direct care coordination to enhance client and family engagement in services. Provides initial screening of client needs and assists the client in connecting to needed resources Answers and screens telephone and email inquiries in a professional and courteous manner to provide clear and accurate program informationResponsible for engagement activities (e.g. communication with external partners, supports appointment scheduling/re-scheduling, in-person, telehealth and/or community visits with client and family, monitor no-show reports, etc.) to facilitate active connection to services and improve behavioral health outcomes Uses social determinants of health and other screening/monitoring tools to identify client and family needs. Collects program specific information using the electronic health record to document activities and advocacy effortsEstablishes and maintains resource connections with external partners and community organizations, manages referral procedures, initiates and tracks linkage to care including dischargesProvides support with arranging primary care and other health appointments, therapy, and increases access to other resources that promote integrated care and family wellness. Collaborates with interdisciplinary team to reduce behavioral health disparities and increase overall engagement in careConnects client with community services, and provides follow-up and monitoring to ensure continuity of care among internal providers and between internal and external providersFacilitates internal and external collaboration to develop person-centered or family-centered integrated client treatment plansInteracts with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving servicesDocuments and maintains accurate and timely case notesAssists clients with making and coordinating appointments, and obtaining supports necessary to keep appointmentsFacilitates communication between providers within a schedule that meets outlined timeframesDevelops and maintains close working relationships with community service providers
Minimum Qualifications:
Bachelor's degree in a Social Services related field from an accredited college or university AND One year of experience in a Social Services related field OR High school diploma or GED AND Three years of experience in a Social Services related position OR One year of experience at the lower level Social Svcs Tech 2 (SST011) or position equivalent.
Preferred Qualifications
Experience with CareLogic or other ECR/EMR At least 2 years experience working in behavioral health care OR child and family serving organizationBilingual - ability to read, write, and speak in Spanish and English
Requirements/Competencies:
Must be able to lift 20 pounds,Requires long periods of sitting/standing,Must be 21 years of age,Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.Other duties as assigned.
