Nursing Coordinator (Bilingual Spanish) (Education)
POSITION SUMMARY:
As the Nursing Coordinator in our home care agency, you will play a vital role in overseeing the day-to-day operations of the Nursing/Clinical Department. This multifaceted position requires a dynamic individual with excellent problem-solving skills, a commitment to teamwork, and the ability to maintain efficient office operations. Your role involves promoting a collaborative environment, ensuring punctuality, and actively participating in various departmental responsibilities.
JOB DESCRIPTION:
1. Comprehensive Departmental Understanding: Gain a thorough understanding of the daily operations within the Nursing/Clinical department.
2. Problem Definition and Resolution: Demonstrate the ability to define problems, collect data, and act decisively to facilitate resolution.
3. Efficient Computer Functions: Perform various computer functions to enhance communication between departments and contribute to overall office efficiency.
4. Teamwork Promotion: Actively promote and participate in a team-oriented work environment, fostering collaboration among team members.
5. Punctuality and Readiness: Demonstrate consistent punctuality and readiness to commence work at the assigned start time.
6. Routine Daily Responsibilities: Maintain efficient methods to ensure daily responsibilities are carried out routinely and effectively.
7. Incident Reporting: Report major incidents promptly to the Clinical Administrative Supervisor, Director of Patient Services, and Director of Operations.
8. Mission and Values Awareness: Implement awareness of the companys mission, vision, values, and goals in day-to-day operations.
9. Departmental Support: Fill in as needed for others in the Nursing/Clinical department, contributing to seamless departmental operations.
10. Professional Phone Etiquette: Answer phones in a professional and timely manner, providing courteous assistance.
11. Patient Satisfaction Monitoring: Regularly monitor patient satisfaction and promptly inform the relevant departments of any patient complaints.
12. Privacy and Ethics Compliance: Respect patient and employee rights and privacy, ensuring the security of protected information. Practice in an ethical manner and comply with the agencys compliance program and privacy policies.
13. Task Completion: Complete other tasks as assigned by the Clinical Administrative Supervisor, Director of Patient Services, and Director of Operations.
RESPONSIBILITIES:
I. MD ORDERS
a. Review for completion and accuracy of MD Orders.
b. Submit all orders to the physicians office.
c. Track retrievals of all MD orders.
d. Maintain spreadsheet and MD order reports for clinical meetings.
e. Notify DPS if orders cannot be signed.
II. MAILING OF DOCUMENTS
a. Mail SOC packages and plan of care to patient.
b. Track SOC packages if not returned.
c. Maintain SOC/POC Tracking sheet.
d. Mails outpatient satisfaction surveys on a quarterly basis.
e. Liaison between Nursing/Clinical department and field nurses.
III. RN COMPLAINCE
a. Maintain the compliance of active and new hires for clinical staff.
b. Run monthly reports to make sure all RNs are compliant in all areas.
c. Maintain nurses files and upload all documents into HHAExchange.
IV. DISCHARGES
a. Running thedischarge report weekly.
b. Filling in discharge summaries and ensuring the Clinical Supervisor reviews and signs off on each discharge summary.
c. Creating a spreadsheet of discharge summaries
V. CHHA REFERRALS
a. Referring CHHA Services to patients that require special visits.
b. Entering special visit notes and CHHA referrals into HHA Exchange
c. Tracking responses from CHHA and entering the response into HHA Exchange
VI. NUSING PAPERWORK AUDITS
a. Under Supervision of the Clinical Supervisor: review nursing packets.
b. Sending packets to nurses for revisions.
c. Working with the Clinical Supervisor to ensure all visits are completed accurately before placing them into the cleared nursing folder.
VII. RN VISITS ASSIGNMENTS
a. Assigns all RN visits (SOCs, reassessment, special visits).
b. Follow up on RN case load.
c. Liaison with RNs and office for difficult to contact cases.
d. Maintenance of goggle spreadsheet.
e. Special visits as requested- ensuring visits are complete within a 48-hour timeframe.
f. Identifies DTS cases and informs DPS for internal follow up with contract and patients.
VIII. RN VISITS DATA ENTRY
a. Enter cleared completed visits into HHA Exchange calendar, aide evaluation into HHA Exchange compliance, check RN authorization in HHA Exchange and frequencies of visits for accuracy.
b. Transfer complete entered package in the nursing saved package folder.
IX. RN VISITS REPORTS/SOC VISITS
a. Run monthly Nurse visit reports from HHA Exchange (SOCs, Reassessment visits)
b. Transfer report onto the Nursing visit goggle spreadsheet.
c. Enter all new SOCs into Nurses spreadsheet daily.
WORK ENVIRONMENT:Works in an office environment with regular exposure to staff and patient elements and occasional stress.
COGNITIVE REQUIREMENTS:Must work cooperatively with others and perform a wide variety of complex and complete tasks involving office equipment and machinery.
QUALIFICATIONS:
High school diploma or equivalency required. Two years of college preferred.
General office experience is required, preferably in a home care setting or related field.
Proficient in Microsoft Office Suite and HHA Exchange.
Excellent leadership and problem-solving skills.
Able to maintain a scheduling system manually and data entry proficiency.
Excellent organizational skills with attention to detail.
Excellent interpersonal and communication skills.
Investigative ability, highly organized, self-motivated, takes initiative,
Adaptability to pressure.
