The Pharmacist shall be responsible for compliance with federal and state lawsand regulations, drug procurement compounding, dispensing and control,distributing, supervision and management of the pharmacy operation. Theobjective of the Pharmacist is to contribute to the comprehensive health careprogram of the hospital. Job Duties: Monitors drug therapies regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose according to age, weight and diagnosis.Provides pharmacokinetic services and drug dosing for individual patient based on age, weight and diagnosis.Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.Reads, extracts, and interprets information in patient charts.Detects and reports suspected adverse drug reactions.Supports the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.Provides drug information to medical staff, other health professionals, patients and their families upon request.Participates in inventory maintenance and the annual inventories.Performs essential duties of the director in his absence.
